In this area we include additional administrative policies that are important to know when visiting our office.
- A $25 no show fee will apply to all personally scheduled appointments that are not attended, or canceled 24 hrs. prior
to the appointment.
- Our office is required to obtain complete and comprehensive medical histories from all patients regardless of diagnosis.
- Co-pays and/or deductibles determined to be appropriate by your insurance will be due at the time of service.
- Insurance cards and updated insurance information are required prior to every appointment.
- Appointments must be scheduled.
- Same-day add-ons can be accommodated, but should understand that longer waits are to be expected.
